Job Description:
Functions as a member of the interdisciplinary team (IDT),
assisting in the implementation and delivery of clinical services
to patients and families. Provides care to Treasure Coast Hospice
patients as directed by the Registered Nurse. QualificationsHigh
school diploma or equivalent preferred.Must be a Certified Nursing
Assistant from the Florida State Board of Nursing.Must be Certified
Hospice and Palliative Registered Nurse (CHPNA) within 12
months.Must maintain a current CPR (BLS) certification.EMR system
knowledge.Must maintain a valid Florida driver's license.Must
maintain current auto insurance.Must demonstrate clinical
competency upon hire and annually thereafter.Must be able to read,
write, and verbally report clinical information to patients,
caregivers, and other Hospice staff.Must complete 12 hours of CEUs
annually as well as additional company requirements.Must be able to
handle sensitive issues and work well with others.Attendance is a
condition of employment. ExpectationsProvide program development
strategies for Treasure Coast Hospice, in an organized, routine
capacity that will enable the cohesion of team developments. Work
collaboratively with other departments, demonstrate and encourage
leadership for a comprehensive approach to program development.To

AreasUnder the direct supervision of the RN, and following the plan
of care to provide comprehensive care to patients and families
including but not limited to:Responsible for upholding Treasure
Coast Hospice/Model Standards for patient visits and compliant
documentation.Clearly understanding and adhering to each
Standard.Motivate and lead self with confidence, energy, and
integrity.Nurse Assistant must report all concerns about the
patient or family to the RN Supervisor.Perform household services
to maintain a safe and sanitary environment in the areas of the
home used by the patient.Light cleaning and laundering essentials
to the comfort and cleanliness of the patient.Perform bed bath,
sponge, tub, and/or shower bath.Perform hair shampoo, nail and skin
care, oral hygiene for each patient.Ensure safe transfer techniques
and ambulation.Assist patients with normal range of motion and
positioningEnsure adequate nutrition and fluid intake.Follow all
standards, policies and procedures as well as state and federal
regulations.Maintains appropriate professional boundaries with
clients and staff.Maintains confidentiality of patient information
at all times.Plans, organizes and prioritizes work effectively and
efficiently.Ability to handle challenging work environment, while
maintaining a positive and professional image.Performs all job
responsibilities in accordance with prescribed safety and infection
control procedures, including thorough hand washing, use of
disposable gloves where indicated and proper disposal of soiled
material.Fulfill all responsibilities related to the success of the
strategic plan of the organization, as requested and assigned.OSHA
